Look for the “Sign up for our Free Schedule Update Email.”  When you register your email address, you will receive an email alert if a game is cancelled or rescheduled.  Check out the site.

Students who plan to participate must provide the following prior to the first practice for any sport:

1. 1. A Pre-Participation Physical Evaluation Form:  http://www.hcpss.org/athletics/form_prepartphyseval.pdf

– All student athletes are required to have an annual physical examination by a physician after April 1st of the preceding school year and have the form signed by the physician.

1. 2. A completed Athletic Participation Form: http://www.hcpss.org/files/form_athleticparticipation.pdf

-Includes:

·        Parental permission

·        A medical insurance company and policy number covering the sport in which the student wishes to participate.

1. 3. Verification of age – Usually a copy of the athlete’s birth certificate. This is usually only needed for new students and incoming 9^th graders.
2. 4. Verification of residence – required annually as requested: http://www.hcpss.org/schools/enroll.shtml

- Examples:

·        Current utility bill

·        Phone bill (not cell phone)

·        Lease or rental agreement (A post office box is not acceptable)

·        For students listed under the Multiple Family disclosure guidelines, student must be appropriately registered.

1. 5. Wilde Lake Standards of Excellence Form – (Link to form) Signed each season by both athletes and parents/guardians.
2. 6. Two completed medical cards supplied by WLHS.
3. 7. A copy of the most recent report card.
